This form is to be used to:

• Cancel a current garden waste permit and subscription.
• Request a replacement garden waste permit due to loss or damage
• Change the address of your garden waste permit due to a house move
• Report that a permit has been purchased but not received

If you are moving address, please only complete this form once you have moved, as the new permit will need to be sent to the new address.

There is a £5 administration fee per permit when reporting a change of address, a damaged or lost permit or if a permit has been ordered for an incorrect address.

If you are requesting to cancel your permit and require us to remove your green garden waste bin, please complete a Container Removal Request.
