The personal data that you provide will be used to investigate and respond to your complaint and the legal basis for this processing is the Local Government Act 1972.

You must complete this form in order for the complaint to be investigated so that there is sufficient details for the Council to know what has occurred.

If you do not include your personal details the Council may not be able to investigate the complaint and will not be able to report back to you on the outcome of the complaint.

Your personal data will be shared with relevant officers within the Council and details of the complaint may need to be discussed with relevant contractors who have provided a service to you on behalf of the Council.

Your personal data will be kept for five years after the complaint was concluded.

You have the right to access your personal data and to complain if you think your data is being used incorrectly. You may also have other rights, such as to have your data corrected or erased.
