Who can join the PACE scheme?
- Patients with existing selected medical conditions (see below) who do not currently take part in physical activity
- Over 16 years of age
- Motivated to start an active lifestyle
- Patients of any of the local doctors practices listed below
What medical conditions are included in the scheme?
To be referred a patient must have been diagnosed with one or more of the follwoing conditions:
- Hypertension
- Diabetes
- High risk of coronary heart disease
- Lower back pain
- Osteoporosis
- Coronary Rehabilitation
- Obesity - see below

What does the scheme include?
The scheme is designed around 24 exercise sessions over a 12 week period (2 sessions per week) which take place in the local Leisure Centre and include gym activities, swimming, aerobics and water aerobics. Patients purchase vouchers at the leisure centre and use a voucher each time they attend an exercise session. Before starting the programme patients will meet with trained gym staff who will help them chose which activities are best for them and give them advice on how to train and achieve the best from the activities. Trained staff are always on hand to offer advice during the 12 week programme.
Vouchers cost £10.80 for a book of 8 off peak vouchers (day time and weekends) and £15.95 for a book of 8 peak voucers (all other times)

GP Referral Scheme extended to address concerns of obesity
The local GP exercise referral scheme (PACE) has been extended to encourage local doctors to refer patients struggling to lose weight.
Previously, the PACE scheme had included patients referred for conditions including high blood pressure, lower back pain, non-insulin dependent diabetes, osteoporosis and those with a high risk of coronary heart disease. From the start of this year GPs can now target patients for whom weight is a threat to their health.
The PACE scheme is designed to increase patients levels of physical activity within a supportive environment, helped by experienced and qualified leisure centre staff who are aware of the best exercise for the individual patients and will design personal training programmes for each individual.
Members of the public who feel they could benefit from being part of the PACE scheme should talk to their GP or Practice Nurse to find out if their practice is participating in the scheme.
