Application forms are available from the main council offices in Milverton Hill, the Town Hall, Royal Leamington Spa or Warwickshire Direct Warwick, Kenilworth and Whitnash, or you can contact the Electoral Registration Office on 01926 456111. Alternatively you can download a copy of an application form using the link below:

If you have printed out the form, please complete and return it to the Electoral Registration office, Members' Services, Riverside House, Milverton Hill, Royal Leamington Spa, Warwickshire, CV32 5HZ. Unfortunately, you cannot fill in the form electronically as we need a signature from you.
Completed applications for Postal and Proxy Votes must be received in the Electoral Registration Office no later than 5 pm, 6 working days prior to an election.
